On Oct. 29, 2025, Salt Lake City police officers responded to an assault in progress near 2100 S. 300 West.
When officers arrived, they found 47-year-old Jose Hernandez and three other men at a nearby gas station. The three men told officers that Hernandez was having a heart attack and were unable to describe what was happening to the 911 dispatcher.
Officers attempted to speak with Hernandez further to understand what had happened, but he declined medical attention and refused to answer their questions.
While officers were gathering more information, Hernandez began to put rocks in his sweatshirt pouch. One officer told him to put them back, but he stood up and began to walk away.
The officers commanded Hernandez to put down the rock, but he refused and ran away from the officers. He then turned and threw a rock at an officer and the officer fired his gun at him.
Hernandez was taken to the hospital, where he died from his injuries.
